Why Hospital Performance Does Not Predict Home Safety

At the hospital, patient performance is evaluated under controlled conditions.

Activities are structured. Attention is directed. Variability is reduced.

Research in aging and mobility shows that performance under single-task, lowvariability conditions often overestimates real-world function, particularly in individuals with reduced capacity to tolerate stress.

Studies of dual-task walking and divided attention demonstrate that when physical and cognitive demands occur simultaneously, gait stability worsens, reaction time slows, and variability increases. These changes are strongly associated with falls and re-hospitalizations.

At home, tasks are rarely performed in isolation. Movement occurs while navigating variable environments, managing disease-related symptoms, responding to changing conditions while managing multiple tasks.

This is why hospital performance does not reliably predict post-discharge safety.

Discharge is not simply a transfer of care. It exposes the patient to real-world, everyday demand.

Most post-discharge care is often delivered in siloed parts. Exercise, mobility, medications and disease management are addressed separately.

Falls and re-hospitalizations rarely result from a single issue. They emerge when physical limitation, treatment effects, attention, and environmental complexity interact in a patient with limited margin for error.

Preventing the next adverse event requires a comprehensive home-based approach that addresses these interacting contributors, not isolated components.
