When the Numbers Don’t Tell the Whole Story

We’ve talked before about gait speed as a sixth vital sign.

A new 2026 study gives us another reason to pay attention to it, especially when the rest of the clinical picture looks reassuring.

Researchers studied more than 4,100 community-dwelling older adults, including patients with both heart disease and diabetes. These are patients we monitor closely: blood pressure, A1c, medications, labs, symptoms.

But one simple measure helped identify who was headed toward hospitalization:

How fast the patient walked.

Gait speeds below 0.58 m/s for men and 0.68 m/s for women were associated with increased one-year hospitalization risk.

And the longer-term numbers were striking. Among men below the gait-speed threshold, 23.7% died within three years, compared with 12.6% above it. Among women, mortality was 16.1% versus 9.4%. (Hong et al., 2026, Physical Therapy)

That’s what makes this finding important. These weren’t patients with an unknown diagnosis. Their diseases were already known, measured, and treated.

The point isn’t that gait speed matters. We already knew that. The point is that medical stability and functional stability are not the same thing.

We may be successfully managing what the patient has while missing what is happening to the patient.

A1c can be controlled. Blood pressure can be controlled. Medications can be optimized. No major event may have occurred.

And the patient can still be getting weaker, slower, and less able to function. Disease markers tell us about the disease.

Function tells us what the disease is doing to the person.
