The Trend Matter More Than the Score

Two patients can have the same performance today, but very different trajectories.

When evaluating older adults, it’s natural to focus on where they are today.
Are they independent? Can they still walk safely? Do they need more help than
before?

These are important questions. But emerging research suggests an equally
important question often gets overlooked: How have they changed over time?

A longitudinal study of well-functioning older adults found that even among
individuals with similar gait speeds at a given point in time, those on a trajectory of
faster decline were at significantly greater risk of adverse outcomes. The finding
highlights that how function changes over time may be as important as the
measurement itself (White et al., J Gerontol A Biol Sci Med Sci., 2013).

In other words, two patients can appear very similar today but have very different
futures.

One has remained stable for years. The other has steadily declined over the past six
months.

The snapshot is the same. The trend is not.

This is why seemingly small changes deserve attention. Needing a little more time to
stand, walking a little slower through the hallway, or requiring a little more
assistance than six months ago may not seem dramatic on their own.

Collectively, however, they can reveal a meaningful change in functional trajectory.
Functional decline rarely occurs all at once. More often, it develops gradually until
the cumulative change becomes impossible to ignore.

The most important question may not be, “How is this patient doing today?”

It may be, “Are they doing better, worse, or the same as they were six months ago?”
