The Risk Tier We Often Don’t See Early
In Many older, higher-acuity populations, ~25-30% are already in moderate to severe frailty, the group with the highest rates of hospitalization, falls, and repeat events. That group is generally easier to recognize.
The challenge is what comes before it.
There is a much larger group of patients who are still independent and living at home, but beginning to lose capacity. Not enough to stand out or trigger concern, but enough to change what happens next.
Nearly, 1 in 3 older adults are already in early-stage frailty. This is where patients are transitioning.
In most cases, care begins once risk becomes obvious, after a fall, an ER visit or a hosprtalization. By then, the pattern is already established. Recovery is slower, events begin to repeat, and the margin smaller.
But that pattern didn’t start there. It developed earlier, in the phase where changes are subtle and easy to miss.
That is the opportunity.
Not after patients become high-risk, but while they are becoming high-risk. Because once they arrive in the higher tiers, we are often managing what has already progressed. Earlier, we still have the chance to change thr trajectory.
