The Hidden Risk: The Gap Between the Clinic and the Home
In rehabilitation, there is often a gap between capacity and performance – what a patient can do in a controlled setting versus what they can do in daily life.
The clinical picture is often incomplete. Falls may go unreported, subtle balance changes are minimized, and medication use is typically assumed to be consistent. They walk short distances, transfer without assistance, and complete tasks without obvious difficulty. But clinic performance does not always reflect real-world function.
Clinic environments reduce demand:
- Short distances
- Predictable surfaces
- Minimal obstacles
- One task at a time
Daily life is not.
Real-world function requires repeated movement, navigation of obstacles, and managing multiple tasks at once. Research in older adults has shown that gait and stability decline as cognitive and environmental demands increase, even when baseline performance appears adequate (Verghese et al.).
There is also a difference in how patients perform when observed. They tend to concentrate more, move more deliberately, and avoid risk during an exam. In practice, the visit often reflects a patient’s best-case performance, not their typical day.
This is why small compensations matter:
- Using hands to rise from a chair
- Pausing before initiating movement
- Relying on nearby support
Individually, these may seem minor. But studies of functional performance show these changes often precede more significant decline and are associated with increased fall risk and loss of independence (Studenski et al.).
When these signs are present in a controlled setting, it often means the margin for safe function at home is already limited.
