TRADITIONAL HOME HEALTH DOESN'T GO FAR ENOUGH
Ever wonder about the assumptions you have to make about your senior patients?
Are they taking their medications properly?
Are they effective (what do their vitals look like the other 360+ days they aren’t in your clinic)?
What safety risks do they face at home that are invisible from the clinic?
Are they getting all of their self care and other activities met SAFELY and CONSISTENTLY?
Traditional home health care is not enough to meet the needs of the majority today’s aging seniors. Approximately 1 out of 6 seniors return to the hospital within 30 days of discharge and many will return over the next several months. Current clinical models of care and training primarily focus on managing the acute illness and/or injury, and most clinicians are unaware of the 3 components to an effective strategy to manage chronic conditions. This leaves many patients without the necessary systems in place to reduce the risk of future problems, costs and burden on them and their caregivers.
