A Shrinking World IS a Clinical Sign
We know the diagnoses. We track labs and other
clinical signs.
Yet some of the clearest evidence of risk may be
showing up outside the exam room.
A recent systematic review of 17 studies involving more than 80,000 adults found
that impaired physical function was consistently associated with hospital
readmission. The researchers specifically noted that conventional medical markers,
including lab values and chronic-condition counts, may not fully capture that risk.

One measure stood out: life-space mobility.
Not simply can the patient walk? Where do they actually go?
Life-space measures how far someone moves through their everyday world, how
often, and how independently.
In one study of patients with CHF or COPD, restricted life-space was associated
with approximately 70% higher odds of readmission within 90 days. (Fathi et al.,
2017, Journal of the American Geriatrics Society)
Think about the patient who still walks independently and manages basic ADLs. Six
months ago, they went to church, shopped, ate out, and visited family.
Today, they mostly move between home and medical appointments.
Nothing dramatic happened. But their world contracted.
That’s the signal.
We often wait for decline to become obvious: a fall, hospitalization, ADL loss, or
abnormal finding.
But risk may already be revealing itself in something much simpler:
The places they no longer go. Sometimes the patient’s world gets smaller before
the medical record tells us why.
