15 Seconds to Predict Frailty and Fall Risk

We’re big believers in proactive care. How do we help identify at-risk seniors before a crisis or injury occurs? Some assessments can take several minutes. This one takes less time than logging into your laptop.

One of the simplest, yet most powerful ways to predict frailty and fall risk is the 5 Times Sit to Stand Test (5xSTS). It requires no special equipment—just a chair and a timer. The test measures how quickly someone can stand up from a chair and sit back down five times in a row, without using their arms.

Why does it matter? Because the ability to rise from a chair reflects leg strength, balance, and overall physical function—key factors in maintaining independence. It is also associated with unmanaged comorbidities.

For adults ages 70–79, the benchmark is completing the test in 12.6 seconds or less. For those 80–89, the goal is under 15 seconds. Falling outside these ranges doesn’t just mean slower movement—it may signal underlying frailty and higher risk of decline.

The numbers are striking:

* Seniors unable to complete the test are over 4 times more likely to fall and 25 times more likely to develop disability.
* Frail seniors, in general, are 2–3 times more likely to fall than their non-frail peers.

The good news is that performance on the 5xSTS can improve with regular practice, strength training, and mobility exercises. Just as importantly, catching delays early gives families, caregivers, and healthcare providers the opportunity to intervene before a crisis occurs.

Fifteen seconds can tell us a lot—and sometimes, it’s the difference between independence and decline.
